Application Forms
With the exception of the graduation forms, please submit all forms to the office of Engineering Student Services located in Engineering Building III, ENC1302. Forms may also be submitted via fax or e-mail to an advisor.

• Student Information Form: Part 2 of the College Application
• Department Application (for current Engineering students only)
  For Chemical, Civil, Computer, Electrical, Industrial, or Mechanical Engineering Programs

• Late Drop/Add Petition Form
• Instructor’s Documentation Form
• Medical Form
• Reinstatement from Academic Dismissal Form
• Advising Record for Academic Renewal I and II (ARI and ARII)

DIRECTIONS: Please open or download either the Bachelor Degree Graduation Application or the Graduate Degree Graduation Application and the appropriate Certification Check List. Fill out the highlighted areas on these two forms, print the forms and submit them to your departmental office for approval.
